Is this the first time the nominee has been nominated for the All-USA Community College Academic Team/Coca-Cola All-State Community College Academic Team?
Does the nominee understand that all contents of the nomination form are subject to rigorous fact-checking, and that you may be disqualified if misstatements are found?
Does the nominee understand that all contents of the nomination form may be released to the public through national media?
Does the nominee understand that any suspension, probation or other serious disciplinary action between the time of this nomination and the AACC Convention on April 17-20, 2010, should be reported to Phi Theta Kappa and may result in possible disqualification?
