All-USA Community College Academic Team Eligibility Survey

Nominations are now open for the 2008 All-USA Community College Academic Team. The 20 members of the First Team will win trophies, $2,500 cash awards, and have their accomplishments noted and photographs published in USA TODAY as representatives of all outstanding two-year college students.

The following is a survey to determine your eligibility to be nominated for this program.


Eligibility Data
  1. Will the nominee have at least 45 semester (or 60 quarter) college-level credit hours either completed at or transferred to a community college by December 31, 2007, and be on a pace and intend to have at least 53 semester (79.5 quarter) college-level credit hours by August 31, 2008?

  2. Will the nominee have a minimum of 30 semester (45 quarter) college-level credit hours completed at a community college in the past five years?

  3. Is the nominee on track to obtain an associate or bachelor degree?

  4. Does the nominee post a minimum cumulative grade-point average of 3.25 out of a possible 4.0 in all college credit coursework completed in the last five years? (Include coursework at all institutions attended in the past five-years when calculating GPA.)

  5. Is the nominee’s record at the community college free from suspension, probation or other serious disciplinary action?

  6. Is the nominee’s record either 1) free from any criminal conduct or 2) if a convicted felon, has completed all conditions or sentencing, including probation?

  7. Is this the first time the nominee has been nominated for the All-USA Community College Academic Team?

  8. Is this the only All-USA Academic Team program to which the nominee is applying this year? (Other programs include the All-USA Four-Year College Academic Team and the All-USA High School Academic Team.)

  9. Does the nominee understand that all contents of the nomination form are subject to rigourous fact-checking, and that you may be disqualified if misstatements are found?

  10. Does the nominee understand that all contents of the nomination form may be released to the public through national media should you be recognized?

  11. Does the nominee understand that any suspension, probation or other serious disciplinary action between the time of this nomination and the AACC Convention on April 7, 2008, should be reported to Phi Theta Kappa and that this may result in possible disqualification?
