Appendix 1: Membership Application

Online Membership Application enables an eligible student to complete an application and pay their membership fees online. If the advisor chooses to use this functionality within PAM, they will send an invitation to the student with the link and Passcode the student will need to process their application online. If the student chooses to use the online application, they will go to www.ptk.org/acceptmembership/ and enter the Passcode that will be assigned to them in PAM.



When an advisor allows their students to accept membership into Phi Theta Kappa using online membership application, each eligible student will be assigned a Passcode. This Passcode can be viewed on the Edit Eligible Student page within PAM. If advisors export a file from PAM to send letters or emails to students, the Passcode will need to be used in this correspondence.

When a student goes to www.ptk.org/acceptmembership/ they will enter the Passcode that was included in their invitation to Phi Theta Kappa. Here the student can choose to Learn More about the chapter or Accept Membership.
If the student chooses to Learn More, they can view specific information about the chapter at their college. This information provides them with the name of the advisor, a list of the chapter officers, scholarships that are currently available, as well as other important information about the chapter and Phi Theta Kappa.
If a student chooses to Accept Membership they will be routed to the following screen:

From this screen, students will be able to verify their demographic and graduation information. They will also have a chance to read the membership oath and agree to the requirements of being a member of Phi Theta Kappa. Here also, the student will pay for their membership into Phi Theta Kappa using a personal credit card.

When the student selects the Submit Payment button they will see the screen below.

When the New Member’s credit card has been approved, the student will receive their Phi Theta Kappa Member ID and an Invoice Number for the credit card transaction as indicated on the page below. This page will also request some additional information to be used for scholarships and other membership benefits.

When the additional information is submitted, the page below will appear:

This is the final page in the Online Membership Application process. This receipt page contains the Member Number, Invoice Number and Amount of Payment. The Get Connected link at the bottom of the page will route the student to the official Phi Theta Kappa website. The student will be able to sign-in using their Member ID and view benefits tailored especially for Phi Theta Kappa members.
