Information on completing the online Chapter Annual Report will be sent February 27 to one advisor per chapter. Annual Report forms will only be mailed to advisors who have not submitted online reports by April 30. The form will be available from the online Advisor Corner, www.ptk.org/advisor/ in early March. Annual Reports must be submitted to Phi Theta Kappa Headquarters by May 14. According to the Phi Theta Kappa International Constitution, a chapter must submit an Annual Report each year to be considered in good standing.
