Conference Registration Application
The registration form should include these items:
- Registration Fee and Deadline
- Chapter Name
- College Name
- Campus Name (if applicable)
- Number of People Attending (including # of advisors and members)
- Name of People Attending (if you want to prepare nametags in advance)
- Email Address, Mailing Address and Telephone Number for Chapter
- Federal ID Number
- Special Needs of Attendees (dietary, accessibility, etc.)
- Method of Payment
- Checks Should Be Made Payable To: ________________
- Mailing Address for Registrations To Be Sent
- If Applicable, Name of Chapter's Voting Delegate
- No Tolerance Policy*
- Cancellation Policy
- Any additional information needed by host chapters
*Sample "No Tolerance" Policy: Alcohol and unlawful controlled substances will not be permitted at any Phi Theta Kappa official conference events. Conference participants using alcohol or controlled substances will be asked to leave the conference immediately.
