Important Information about this form:

THIS FORM MUST BE COMPLETED TO OFFICIALLY REMOVE FROM MEMBERSHIP any Phi Theta Kappa member who does not meet the academic maintenance requirements for continuing membership in the Society as outlined in the local chapter's bylaws (approved by Headquarters) and in the International Constitution (Article IV, section 3).
