  • $3,000 calendar year deductible (Up to $2,000 reimbursed after employee pays $1000)
  • Maximum Out-of-Pocket in 2014, $6,350 (Up to $1,350 reimbursed after employee pays $2,000)
  • $25 - $40 co-pay for office visit
  • Network Provider 80% - 20% coverage

  • Unlimited lifetime maximum benefits

*Prescription Drug Coverage

  • $100 calendar year deductible
  • $15 co-pay - Category 1

Annual Leave/Sick Leave

Exempt employees:

  • Annual Leave - 11.25 hours per month
  • Sick Leave - 7.5 hours per month

Non-exempt employees (based on compensable hours):

  • Annual Leave - .07 per hour
  • Sick Leave - .05 per hour


Educational Assistance Program

  • Employees are eligible following two years of service. 100% of tuition and fees are paid by Phi Theta Kappa for approved coursework. Payment is limited to one course per academic term. Employees who make use of this plan are required to remain in its employment for at least one year after completing coursework paid for by the Organization.
  • 5% per year tuition discounts and waived application fees at Colorado Technical University.
